Heart 2 Heart: Compassionate Conversations with the Ones We Love -A learning session on the benefit of Palliative & Hospice care and the importance of Advance Care Planning
Presented by Dr. Jade Nelson, FNP-C, Aftan Garrett, AGNP-C, and Britannia Tate, FNP-C
Location: CN Jenkins Church
Date/Time: February 17th 2024 10-12 PM
Are you living with a chronic or serious illness or have a family member/friend that may be affected? Have you completed an Advance directive?
Please join us to learn more on the benefits of Palliative and Hospice care as both are specialized medical care for people living with serious illnesses. The goal of both services is to improve quality of life for both patient and families. Palliative care services are provided along with curative treatment. The focus of care is providing relief from symptoms and stress related to the illness. Hospice care is appropriate when there are no more curative treatment options available. This care is focused on pain and symptom relief for a person at end-of-life.
The purpose of this session is to educate our community on supportive care services available while dealing with serious illnesses. There are known disparities in healthcare among minorities. Unfortunately, this is also seen within Palliative and hospice care. Studies consistently document lower rates of hospice use for minority older adults. Among Medicare beneficiaries who died in 2010, 45.8% of whites used hospice compared to 34% of African Americans. Also, a study at John Hopkins found that 34.9% of Black study participants who died used hospice services over the study period, compared with 46.2% of white participants. A contributing factor that has led to this disparity is lack of knowledge of these services.
In addition, participants will have the opportunity to complete an Advance directive which is a legal document identifying a Healthcare Power of Attorney and Living Will that list treatment preferences during a significant decline in health.
Overall, we hope this learning session will educate and initiate conversations with loved ones about your goals of care during such a vulnerable time as the worst time to make such important decisions is during a time of stress and crisis.
